PART-TIME HOMEMAKER:

Rendezvous Pointe is looking for a compassionate, kind-hearted and patient person, who loves to work with seniors.

Our Homemaker position will assist seniors with housekeeping, shopping, laundry, essential errands, maintaining a safe and sanitary environment and basic meal preparation and planning.

Other duties will be:
1. Evaluates and reacts appropriately to each senior
2. Completes orientation and on-going training as directed
3. Maintains confidentiality
4. Communicates and works with the seniors in a respectful manner
5. Completes all assigned tasks while staying within the care plan time limit

Pay: $15.00/hour 

ACCESS CARE COORDINATOR:

Rendezvous Pointe is looking for a compassionate, loving, hardworking Access Care Coordinator.

This position will work closely with seniors. Any education in healthcare, elderly care, health management, or related fields are appreciated, but not required.

The job responsibilities include, but are not limited to:

Assist in the management of the WyHS Program to serve the elderly and disabled citizens of Sublette County and help them remain in their own homes and prevent premature institutionalization.

Provide case management tools, a process by which an individual’s needs are identified; social, homemaking, personal care, respite and medical. Locate the service areas to meet those needs. After they are identified, services are coordinated and monitored.

Provide guidance to all homemakers and C.N.A.’s.

Assist with general office duties by answering the phones and assisting with the lunch registration when needed. Other duties are listed but not limited to; helping seniors make copies, looking up information, scheduling rides or appointments, and taking blood pressures.

Hours: 20 hrs. Week with the possibility of increased hours if the program grows.
